New Starter Documentation

Once school has confirmed your offer of place at Derwent Primary School, the following documentation is compulsory and will need to be completed and returned by the agreed deadline.

(please note that for September 2026 starters, wrap around care will be available to book from 1^st September)

If your child has any medical conditions or dietary requirements that you have notified us of, additional forms will be need to be completed. They are attached below for your information.

Any completed forms will need to be emailed to the school office to derwent@derwentschool.co.uk for the attention of Miss Ferris. If there are any issues with completing the documents electronically, please contact the school office.
